7. Land line no of applicant and company
8. Email id of applicant and company
9. Authorisation letter as per page 4 of the form with name of applicant and stamp of organization on the letterhead of organization.
10. Attachment will include any one of the following verifies by CS/ Director/partner of organisation
11. Cert of incorporation
12. MOA and AOA
13. Registered Partnership deed
14. Valid business license document

And one from the following:

1. Annual return
2. IT Return
3. Statement of income
4. Bank details of organization
5. The form must be also signed on the second page (Signature of applicant) and across the photo also.
6. IEC Certified Photocopy (Required only for  DGFT  DSC)

    Note :  The form must be signed by the applicant with stamp on all pages of the form.
